Requirements:
Languages: Candidates must have knowledge of the English Language
Education: Candidates should have completion of Master’s Degree in Social Worker
Experience: Candidates should have two years recent related experience providing care through a holistic mental health, substance use and primary care lens
Physical Requirements:
- The candidates should possess a combination of education, training, and experience relevant to the position
- The candidates should be able to conduct mental status exams and suicide risk assessments effectively
- The candidates should demonstrate the ability to work independently and collaboratively as a member of a multidisciplinary team
- The candidates should demonstrate the ability to establish workload priorities
- The candidates should show the ability to adjust the schedule to deal with unexpected situations
Other Requirements:
- The candidates should have current full registration with the British Columbia College of Social Workers, possess a valid BC Driver’s License
- The candidates should possess knowledge of the principles and practices of a client and family-centered recovery model in mental illness, provide care coordination to a select caseload of clients
- The candidates should understand appropriate acts such as the Mental Health Act, Adult Guardianship, and Public Health Act as they relate to the rights and obligations of clients and staff
- The candidates should demonstrate knowledge and experience in providing culturally safe and competent care, particularly pertaining to Aboriginal people, in Trauma-Informed Practice, Harm Reduction, Recovery-Oriented Care, healthcare disciplines and their role in client care
- The candidates should have broad knowledge of crisis intervention and supportive counseling skills, mental health illness, treatment, substance abuse, addictions treatment, facilities and community resources
- The candidates should be proficient in providing care planning, supportive counseling, crisis intervention, and case coordination and ability in dealing with a variety of situations and responsibilities requiring initiative, creativity, and professional judgment
- The candidates should exhibit effective communication skills, both orally and in writing, with clients and their families, colleagues, physicians, and other healthcare staff, both one-on-one and in groups
- The candidates should demonstrate listening and information-seeking skills that promote communication and lead to a cooperative approach to problem-solving within a multidisciplinary setting and problem-solving skills and use critical thinking effectively with conflict situations
- The candidates should be proficient in operating related equipment and the physical ability to perform the duties of the position and provide consultation and leadership
- The candidates should have basic computer literacy to operate a computerized client care information system and word processing Internet and email software
Responsibilities:
- The candidates should be able to work to address client and family-centered care needs, supported by an integrated health service team and delivery model
- The candidates should be able to collaborate effectively to coordinate a plan of care across multiple sites, clinics, and services within an integrated health service delivery model and interdisciplinary care team. This includes primary care, mental health and substance use services, private General Practitioner (GP) partners, and other Community Care Services
- The candidates should be able to care for clients and families experiencing health challenges such as end of life care, chronic diseases, mental health and substance use issues, and those adversely affected by social determinants of health. They should be able to coordinate a plan of care and support transitions between services
- The candidates should be able to use health management and self-care management principles to identify problems, address issues, or variances from the plan of care in collaboration with the entire interdisciplinary team and other health care services involved
- The candidates should be able to act as a resource for the DTES integrated team, providing information and support based on theory and practice related to clients with complex health care
- The candidates should be able to provide direct care activities such as psycho-social assessing, planning, implementing, evaluating, and documenting client care as part of the Integrated Care Team and collaboratively with other Community Care Services
- The candidates should be able to establish, maintain, and enhance therapeutic relationships based on respect
