Requirements:

Languages: Candidates must have knowledge of the English Language
Education: Candidates should have completion of Baccalaureate degree in Nursing
Experience: Candidates should have a minimum of five years’ experience in the relevant field

Physical Requirements:

  • The candidates should be able to occasionally move and lift objects between 10 – 30 lbs and push files, charts and equipment
  • The candidates should be capable of constantly sitting to perform work on a computer, including research, writing policies, procedures, manuals, preparing presentations and reading and preparing documents
  • The candidates should regularly consult and interact with other departments, units and hospitals to coordinate timely patient, client, resident flow and to communicate regarding division and unit issues
  • The candidates should be able to regularly stand or walk to perform audits, give presentations, respond to patient, client, resident complaints and collaborate with the healthcare team to resolve issues
  • The candidates should regularly perform fine finger and precision work on the computer or with machines that require accurate control and steadiness
  • The candidates should have visual concentration required to view and report on documents, assess workplace situations, and monitor the workplace regarding health, safety and risk management issues
  • The candidates should be able to work with residents in a fast paced environment and ability to work independently

Other Requirements:

  • The candidates should have graduated from an approved and recognized post-secondary nursing education program, or equivalent education or training
  • The candidates should hold current and continuing practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) and professional organizations and special interest groups, current BLS Certification and certification in gerontology
  • The candidates should have experience to working in a geriatric setting with primary responsibility caring for patients and residents with chronic and complex
  • The candidates should possess knowledge of long-term care practices and continuous quality improvement concepts and resident centered care philosophy
  • The candidates should demonstrate a resident-centered care approach with residents and their families
  • The candidates should demonstrate competency in organizational, leadership, interpersonal, team-building, communication, teaching, conflict management and decision making skills
  • The candidates should demonstrate the ability to develop and maintain successful working relationships and progressive leadership
  • The candidates should show evidence of active involvement or willingness to participate in continuing professional education development
  • The candidates should have the ability to facilitate staff and student clinical initiatives effectively
  • The candidates should have a satisfactory record of work performance

Responsibilities:

  • The candidates should be able to function in a leadership capacity as a Registered Nurse IIC Care Facilitator, coordinating the daily activities of the units
  • The candidates should be able to consult and coordinate with other departments, units, and program areas, such as acute care and emergency departments, regarding complicated health issues, complex admissions, and readmissions
  • The candidates should be able to be accountable for the continuity and coordination of resident care, utilizing clinical judgment and critical thinking skills
  • The candidates should be able to assist with the development, implementation, and evaluation of resident quality and safety initiatives, ensuring a high standard of resident care is maintained
  • The candidates should be able to evaluate the effectiveness of resident care through mechanisms such as team conferences, ongoing feedback with team members, residents, and family, and review of staff assignments
  • The candidates should be able to participate as an active team member and act as a liaison for team members in the interdisciplinary provision of care
  • The candidates should be able to facilitate and coordinate resident/family care with other disciplines, departments and programs
