Requirements:
Languages: Candidates must have knowledge of the English Language
Education: Candidates should have completion of Bachelor of Social Work or Registered Nurse
Experience: Candidates should have experience providing case management
Physical Requirements:
- The candidates should have a strong ability to collaborate and work within a multi-disciplinary team
- The candidates should have exposure to pets, second-hand smoke and willing to travel within Whitehorse
- The candidates should be comfortable with latex products, flowers, plants, and other irritants while in client homes or within facilities
Other Requirements:
- The candidates should have experience in developing person-centred care plans and experience in completing assessments in their respective field
- The candidates should possess excellent organizational, interpersonal, and conflict management skills
- The candidates should demonstrate effective oral and written communication skills
- The candidates should exhibit strong skills in working with various cultural backgrounds and demonstrate the ability to take a reflective approach to their practice
- The candidates should have a Vulnerable Sector RCMP security clearance and Class 5 Driver’s Licence and TB screening
- The candidates should have WHMIS 2015 certification and current registration with a Canadian regulatory body

- The candidates should be able to coordinate person-centered care for residents, ensuring it is holistic, continuous, culturally sensitive, and compassionate
- The candidates should be able to value and integrate resident and family experiences into the care coordination process
- The candidates should be able to engage in health care partnerships with residents, prioritizing their assessed needs, values, priorities, and goals in care planning and implementation
- The candidates should be able to respect residents’ dignity, individual freedom, choice, and lifestyle in all actions and decisions
- The candidates should be able to promote meaningful and quality living experiences for residents through their care coordination efforts
